This page explains how to order and license already-published articles for the personal use of interviewed persons or other limited uses. To order, you must know the Keith Risler Article Code for the article in question. Normally this will have been provided to you previously.

TO ORDER: Complete this form and forward with payment by cheque or postal money order, payable in Canadian funds. U.S. and overseas customers may pay in the U.S. funds equivalent applicable when the order is submitted. Due to widely fluctuating currency exchange rates we reserve the right to invoice customers for additional amounts reflecting variations in exchange rates that occur before orders paid in U.S. funds are received.

AVAILABILITY: Some articles may not be available to be licensed for publication, web site or other uses due to prior licensing arrangements. If this is the case at any time when an order is received, you will be so advised.

Orders are supplied only on ISO 9660-format CD-R discs, readable on Macintosh, Windows and many UNIX systems with up-to-date CD-ROM drives.

CANADA: Federal Express, 1 CD-R  disc to most Canadian destinations: Add $20.00. Regular postal mail for 1 CD-R disc: $6.00 per order or CD-R disc. For FedEx shipments to that exceed $20 we will require additional payment and will contact you before shipment. FedEx will not ship to post office boxes, a street address is required.
UNITED STATES: Regular postal mail: $12.00 per order or CD-R disc. Customer is responsible for all import, brokerage or other costs that may apply upon import. Please call for FedEx shipping costs to U.S.A.
OTHER COUNTRIES: Regular postal mail: $20.00 per CD-R disc. Customer is responsible for all import, brokerage or other costs that may apply upon import. For shipping costs that exceed $20 we will require additional payment and will contact you before shipment. International FedEx delivery is available. Call for pricing quote.

TAX PAYABLE (Calculate based on Line 2 "Sub-Total"):
Canadian customers in GST-applicable provinces must add 7% GST on Line 3.
Canadian customers in HST-applicable provinces must add 15% HST on Line 4.
Ontario customers must add 8% Ontario retail sales tax on Line 5.
If you are tax-exempt in any category exemption certificates must be supplied if required by law, or indicate exemption status otherwise.

PURCHASE ORDERS: Corporate purchase orders are accepted for orders within Canada for purchases under $700. Payment is due upon receipt for such orders. Orders for work to be published on a pay-on-publication basis are not accepted.

All pricing as stated above excludes applicable taxes. For all 001-Code articles, all prices will be significantly higher. Prices and terms subject to change without notice; the terms that apply to your order are those that are/were posted here at time of your order. Price quotes when provided are valid for 30 days only and are subject to continued article availability; due to licensing of articles, availability of articles may change on an immediate basis without notice. We do not sell all rights to materials but license only specific time- and market-limited uses. All Adobe Acrobat electronic documents include author information. We reserve the right to decline licensing of articles at our discretion for any reason.
